Día Mundial de las Hepatitis virales
El pasado 28 de julio se conmemoró el “Día MundialContra la Hepatitis”, enfermedad en su mayoría silenciosa que es causada por los virus Hepatitis B y C., que pueden causar manifestaciones agudas como una falla hepática fulminante cuadro muy grave y con elevada mortalidad -, pero la gran mayoría de las veces son crónicas, como loson la aparición de fibrosis en el hígado -una remodelación de su “arquitectura”y la cirrosis, etapa final de la fibrosis, además del riesgo aumentado de padecer un cáncer hepático. Según registros internacionales, la enfermedad causa la muerte de aproximadamente 1,4 millones de personas al año. Pese a esto, suelen pasar desapercibidas. La Hepatitis B es una enfermedad causada por un virus ADN, cuyo único huésped es el humano. Persistehasta 7 días en el ambiente y es estable en superficiesinertes hasta por un mes. El periodo de incubación es de 6 semanas a 6 meses, con un promedio de 2 a 3 meses. Los principales mecanismos de transmisión sonla vía sexual (sin protección), madre a hijo y contacto con fluidos contaminados con el virus. En comparación con laHepatitis C, este virus es más probable que cause hepatitis aguda causando ictericia -el tinte amarillento de la piel-, y otras manifestaciones como malestar general, dolorabdominal, náuseas, vómitos y fiebre. También puedeproducir un cuadro mucho más grave como es la falla hepática fulminante. En Chile esta enfermedad se concentra en personasjóvenes, principalmente entre 20 a 39 años conpredominancia en hombres. La Hepatitis C es una enfermedad causada por un virus ARN, frecuentemente asintomático, por lo cual, la mayoría de las personas la desarrollan a largo plazo de forma crónica, pudiendo llegar a la cirrosis.
El principal mecanismo de transmisión es por vía sanguínea, siendo uno de sus principales factores de riesgo la transfusión de sangre antes del año 1996 (desde ese año en Chile se examina la sangre a transfundir para determinar que no tiene estos virus). Otros son el uso de drogas inyectables, exposición a procedimientos (como inyecciones y diálisis) y en una menor proporción, las relaciones sexuales noprotegidas y la trasmisión de madre a hijo. En Chile se concentra en personas mayores de 40 años, sin mayor diferencia por género.
Por tanto, es fundamental llegar a realizar un diagnóstico temprano a través de exámenes de sangre, siendo deseable que en todas las personas se incluya su búsqueda estando o sintiéndose sanos, más aún si hay alguna evidencia de enfermedad hepática.
En el caso de la infección por virus B, se puedeprevenir eficientemente mediante vacuna para personasseleccionadas por sus factores de riesgo, y para ambos virus existe tratamiento farmacológico muy efectivo, acción que puede determinar la curación de la enfermedad, especialmente en el caso de la Hepatitis C.
Para ello, desde el año 2011, en Chile se cuenta con un Plan estratégico de Prevención y Control de las HepatitisB y C, patologías que son de notificación obligatoria y queestán incluidas en el Plan GES.
En esta línea, desde el 2019 se dispone de vacunación anti hepatitis B a todos los recién nacidos, la que se suma a la calendarización habitual de esta vacuna en el ProgramaNacional de Inmunizaciones junto a la estrategia, a partirde 2021, de iniciar la búsqueda universal de hepatitis B en las embarazadas. Para Hepatitis C, se han realizado grandes avances en relación al tratamiento, ya que se cuenta con antiviralesde alta efectividad, con bajas reacciones adversas, fáciladministración vía oral y corta duración de tratamiento. En definitiva, los datos del resultado de las terapiasen Chile, dan cuenta de un 96,6% de Respuesta Viral Sostenida (RVS), es decir, la eliminación del virus del organismo.
